REALM believes that the future of elder care rests in integrated services – a comprehensive, new approach to health, wellness, life management, care coordination, and delivery.

Over the past five years, a significant portion of REALM’s practice has focused on the development and implementation of integrated health and wellness models.  Our experience has been that models that enable older adults to remain at home can be redesigned to dramatically improve the consumer experience, improve the quality of life and care, and reduce overall cost.

Organizational Visioning
Designing, facilitating, and implementing strategic visioning sessions that open the organization’s team to the realm of possibilities for the future and aligns the organization around a common vision.

Strategic Planning
REALM blends traditional strategic planning processes with best practices emerging from visioning sessions, large-scale collaboration, and graphic facilitation. This complements the traditional planning approaches with processes that engage stakeholders in a holistic integration of their intuitive, emotional, and intellectual understandings of the organization.

Implementation
Strategy implementation is "the process of allocating resources to support the chosen strategies." This process includes the various activities that are necessary to put strategy in motion, monitoring progress, developing adaptation plans, and ultimately achieve organizational goals and is inclusive of leadership, stakeholders, collaborative partners, vendors and consumers.
